Prolactin increase

Classical antipsychotics raise plasma prolactin concentration by blocking dopamine receptors in the tuberoinfundibular pathway, causing gynaecomastia and galactorrhoea in both sexes, and menstrual disturbances in women. A change to an atypical agent such as aripiprazole, quetiapine or olanzapine (but not risperidone or amisulpride) should minimise the effects. If continuation of the existing classical antipsychotic is obligatory, dopamine agonists such as bromocriptine and amantadine that reduce prolactin secretion may help.
